The Journey for Urology Patients
and their Careers

When a child is newly diagnosed with an illness, a health-related issue or a chronic disease it can cause a great deal of anxiety for parents, family and carers. At the Children’s Urology Clinic, Dr Ani strives to make the process of booking and attending appointments as stress-free as possible.
This page includes links to our clinic forms and important information you may need to make your dealings with the clinic run as smoothly as possible.

Before the First Appointment

- Have your GP or specialist write a referral for your child and send this to your chosen clinic.
- Collect or request any relevant scans or testing already performed on your child. You will need to bring these with you to the first appointment.
- The Clinic Secretary will indicate if you need to complete a Urinary Voiding Diary or the Bladder Voiding Questionnaire (found at the bottom of this page).
- Please complete the Patient Examination Consent Form prior to the first appointment.

Your Child’s First Appointment

Welcome to Children’s Urology with Dr Ani! At your child’s first appointment Dr Ani will review your referral and any notes made by your GP or referring specialists. He will also review any scans or testing done prior to your first appointment.
Your child will then be clinically assessed and any further investigations will be arranged if required. If your child requires a physical examination Dr Ani will explain the process in full and ask you to complete a Patient Examination Consent Form. An individual plan will then be discussed with you and your child to best manage your child’s particular problem.
